2016 SUMMER CRISIS PROGRAM:

Community Action Wayne/Medina will begin the Summer Crisis Program July 1, 2016 and ends August 31, 2016.

Help is available for eligible households trying to cope with the summer heat. The Summer Crisis Program can assist with a one-time benefit up to:

  • $300 for regulated utilities.
  • $500 for non-regulated utilities.
  • Eligible households may also receive a fan and/or air conditioner if the household has not received one in 2013, 2014 or 2015.

* ALL HOUSEHOLDS THAT ARE CURRENTLY ENROLLED IN THE PERCENTAGE OF INCOME PAYMENT PLAN (PIPP+) WILL NOT BE ELIGIBLE FOR THE SUMMER CRISIS PROGRAM UTILITY BILL PAYMENT, but can get an AC and/ or fans if they qualify.

 

ELIGIBILITY REQUIREMENTS:

  • To be eligible for assistance, households must have income at or below 175% of the federal poverty guidelines (see below), applicants must also:
  • Have a member in the household who is 60 years of age or older; or
  • Have a member who has a medical necessity verified by physician documentation

  • A NEW take-a- number system will be used.
  • Customers must BE PRESENT when the number is CALLED.
  • Customers will be seen thru lunch.

  • THURSDAY ONLY: 8:30 AM - 3:00 PM

 

ORRVILLE, WADSWORTH, and LODI:

  • ONE DAY A MONTH - Rotating outreach in these areas. Please call our Wooster or Medina offices for more information.

 

DOCUMENTATION REQUIRED:

  • Photo ID for primary applicant
  • Social Security Cards for all members of the household
  • Documentation from a physician stating someone in the household has an illness that would benefit from continued electric services and/or air conditioning
  • Proof of income for the past 90 days from ALL sources for ALL members of the household (13 weekly paystubs/6 biweekly paystubs, child support, social security benefits, retirement, unemployment compensation, cash assistance from JFS, veterans pension/disability, utility allowance, self-employment-previous year tax return with all schedules)
  • Current gas and/or electric bill
  • Documentation from a physician stating someone in the household has an illness that would benefit from continued electric services and/or air conditioning
  • Landlord’s name, address and/or phone number (Landlord Assurance Form when applicable)
  • If anybody has been helping your household within the past 90 days, you MUST bring a letter signed and dated by those individuals

** Household is defined as EVERYONE living under your roof.

*** All ZERO INCOME HOUSEHOLDS must provide an IRS Tax Transcript. (this can take approximately 10 days) This can be done by:
